New Patient Registration
Stoke Surgery welcomes new patients within a designated area. The surgery catchment area covers the Stoke and St Peters Ward. The boundaries are: to the north; St Levans Road, to the east; Alma Road, to the south; Victoria Park and to the west; Paradise Road and Albert Road.
All new patients are requested to complete a health questionnaire on registering with the practice.
All patients registering at the Practice are allocated a named GP who is responsible for their care. However, you can see any doctor of your choice at the Practice. If you wish to be registered with a specific doctor please request this at the time of registration.
All new patients are invited for a routine health check with the healthcare assistant following registration. You will be asked to see the doctor if you are taking regular medication.
Named Allocated GP
All registered patients at the surgery have a named allocated GP. Please contact the surgery if you wish to know who this is. You can request to be allocated a specific GP - please contact us to do this. Please note, you can still see any doctor of your choice at the Practice.
Change of Address
If you are moving house please let the surgery know as soon as possible. Patients moving out of the practice area will be required to register with a practice close to their home address. The doctors do not have to provide home visits for patients who live outside the practice area. Please notify the practice if your telephone number changes.
